CODEAMANI LABS LLC

Reporting capability demo

Solicitation 75D301-26-Q-00146 · Tab 2-2 prototype

A clearer path from “something happened” to a complete report.

This public demonstration shows how CODEAMANI LABS LLC would approach a modernized VAERS-style experience: redesigned landing and navigation, guided branching, inline validation, tooltips, informational popups, and a keyboard-accessible FAQ.

Choose a pathway

Branching starts on the first step. Healthcare providers can submit the PWS vaccine-error report even when no adverse event occurred.

Public / consumer

A member of the public describes who was vaccinated, which example product was given, and what synthetic symptoms followed.

Open the public path

Healthcare provider

Clinician reporting of an adverse event, or a vaccine administration error with no adverse event. Symptom fields hide on the error branch.

Intelligent validation

Required-field coaching, email and lot checks, and date sanity (future dates, birth before dose, onset before vaccination).

Customer-centric help

Tooltips sit next to labels. Informational popups explain why a question exists without leaving the page.

Reactive FAQ

The FAQ control is in the site header on every page. Escape closes it and focus returns to the opener.
